DEAR PATIENT:

Welcome....

At our main offices in Smithtown and Stony Brook, as well as at our satellite offices in Riverhead, Hampton Bays and Mineola you will be welcomed by a caring receptionist who will aid you with the completion of the all those annoying but necessary forms. A certified Physician Assistant (PA-C) or Nurse Practitioner (NP-C) may participate in your care, along with our Board Certified Surgeons and Physicians.

To expedite your visit and avoid delays, we suggest you download the forms necessary for your registration and complete them prior to your first visit.

We will make every effort to have you seen by our doctors as close to the time of  your scheduled appointment as possible, but unforeseen delays in the doctor's schedule may happen. We apologize in advance for those occurrences and are more than willing to reschedule your appointment at no charge if you are unable to wait. In return we request you notify us as soon as possible if you are unable to keep your appointment by calling our appointment desk at:

IMPORTANT NEW PATIENT INFORMATION
If possible, please bring the following when you come for your first visit:
  • All prior medical reports, all actual X-Rays, CAT scan films, MRIs and, if available their accompanying reports. Imaging studies on CDs are welcome.
  • We ask you to complete all forms (found below) at home prior to your visit. If you are unable to complete them, please bring them with you  30 minutes prior to your scheduled appointment and we will assist you.
  • You must bring your health insurance card with you. We have to make a copy for our records.
  • If your insurance requires a referral from your primary care physician, please call that physician for the referral number or bring your paper referral. Lack of a formal referral will result in cancellation of your appointment and need for rescheduling.
  • If your visit is due to an accident, such as a Workers' Compensation or No Fault claim, it is imperative that we have all insurance information prior to your appointment. If you did not have this information when your appointment was made, please call our office prior to your actual visit so that we can verify the information with the insurance company.
  • All fees and co-payments are due at the time of treatment. Please refer to your insurance plan or contract for information on what charges are covered. Any charges that are not covered are the patient's responsibility and are payable at the time of service. Payments are acceptable in cash or by check or credit card.
