C1-C2 Wiring / Fusion (Traditional)  

There are several conditions that can cause instability between the first (C1 or Atlas) and the second cervical vertebra (C2 or Axis). This includes rheumatoid subluxation, os odontoideum and odontoid fractures.

The traditional technique to stabilize these conditions is by way of a posterior wiring and fusion of the arch of C1 and the spinous process of C2. There are several modifications of this classic Brooks/Galli fusion.

In certain cases the head (occiput) has to be included to secure stability (Occipital- Cervical fusion).

Alternative techniques include Interlaminar Clamps, Transarticular Screws and the Odontoid screw.

In elderly patients with major medical contraindications sometimes an external fixation device (HALO) is used.
